DELIVER A HSE-LED INITIATIVE TO IMPROVE THE HEALTH OUTCOMES OF OTHER
EXCLUDED VULNERABLE GROUPS (ROMA, MIGRANTS, WOMEN) WITH INSECURE
AND/OR UNSUITABLE HOUSING CONDITIONS.
LOCATION; initially County Donegal
DURATION; initially 3 months
FUNDING; initially the successful Tender will be awarded the sum of up

Agreement with the HSE. It will be possible to submit subsequent
applications in following years following proof of concept
The initial timeframe, location and funding allocation are hoped to be
extended by this pilot phase proving proof of concept. As such a quick
turnaround for engagement will be that work will need to commence on
September 1st **Apply on the website**.
The proposal aims to work towards a development of prevention led
response to people becoming homeless with a targeted focus on people
who do not necessarily meet the definition of homeless i.e. hidden
homeless communities, with insecure and/or unsuitable housing
conditions, within the excluded vulnerable groups (Roma, Migrants,
Women).
IT IS INTENDED A NUMBER OF ELEMENTS WILL BE INCLUDED, SUCH AS;

website**% linking with GP and Primary Care services
* **Apply on the website** wellbeing plans will be developed to
support self-determined health improvement
* **Apply on the website** support packages will be developed to
assist improved health behaviours
* links to homeless services will be established for an additional

* **Apply on the website**% of all persons will be assigned a key
worker and case management will under pin the delivery of the above
OUTPUTS
* Establish a locally based Task Force in Donegal with relevant
agencies to review models of engagement with those in unsuitable
accommodation
* Introduction of the model of prevention led response to
homelessness and health improvement for those vulnerable groups in
insecure/unsuitable accommodation,
* Have agreed definition of hidden homeless with the interagency
task group
* identify the locations and the health & social needs of the target
cohort.
Monthly reports on these areas will be required, including risks and
issues
